<p>The article discusses the fundamental problem of differential diagnosis of mental disorders in penitentiary institutions. Deprivation of liberty is an extreme stressor that forms a special environmental context in which symptoms, classified under normal conditions aspathological, can act as an adaptive response to abnormal living conditions. The author analyzes the conceptual basis for distinguishing between depressive states associated with a situation of life imprisonment and major depression; paranoid delusions and «prison vigilance» as a learned survival strategy. A methodological approach is proposed that takes into account the environmental context and avoids both overdiagnosis and ignoring true psychopathology. The necessity of revising traditional diagnostic criteria in relation to isolation conditions is substantiated</p>
<kwd-group xml:lang="ru"><kwd>параноидное расстройство</kwd><kwd>нормативность</kwd><kwd>экспертиза</kwd><kwd>пенитенциарная психиатрия</kwd><kwd>дифференциальная диагностика</kwd><kwd>расстройства адаптации</kwd><kwd>тюремная среда</kwd><kwd>депрессия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>penitentiary psychiatry</kwd><kwd>differential diagnosis</kwd><kwd>adjustment disorders</kwd><kwd>prison environment</kwd><kwd>depression</kwd><kwd>paranoid disorder</kwd><kwd>normality</kwd><kwd>expertise</kwd></kwd-group></article-meta></front><back><ref-list><ref id="ref1"><mixed-citation publication-type="other" xml:lang="ru">Заключительная часть акта судебно-психиатрической экспертизы // Инструкция по производству судебно-психиатрической экспертизы: [утв.
